REQUIRED FORMS

The following forms are available and can be accessed and completed via this Webpage; however, they cannot be automatically submitted.  

o        Furlough Certification Agreement Form

Faculty should sign the Furlough Certification Agreement and forward it to their Department Chair.  Chairs should forward this document to their Dean's office.

o        Furlough Dates Agreement Form

Faculty should enter their proposed furlough dates, sign the Agreement Form, and forward it to their Dean's office for approval.
