Change of Name or Address

Policy

A student may change his/her name and/or address at any time. The student will need to obtain the change of name/address form and sign it. If the student is changing his/her name, the form must be accompanied by supporting documentation such as a driver's license, marriage certificate, or a decree name change from a court. Once the form has been signed it must be submitted to the Admissions & Records Office.
