Scholarship Application Information

Please submit the items listed below with your completed application.  Submit your materials to the attention of Cheryl Atienza in the College of Education Dean's Office (COE 1490A). The submission deadline is 5:00 p.m., Friday, February 20, 2009.

To Apply:

1) Review Scholarship Chart:

Please review carefully the Scholarship Chart (link below) to see which one(s) may be available to you.

2) Complete Application:

Download the application form (link below) and fill out completely.  Applications should be typewritten or neatly completed in pen. 

3) Include the following items with your application:

         Transcripts (official or unofficial) that include your last 60 units of college coursework

         A ONE-PAGE Resume that lists your educational background and degrees, honors, awards, activities, and work experience.

         Two Professional Letters of Recommendation

o        At least one of the letters of recommendation is required to be from a faculty member who will discuss your academic and personal achievement and potential for success.  Each letter must be submitted with an original signature

         Personal Statement 

o        Your personal statement should be one to two typed pages, double-spaced, and should address the following:

         Statement of need

         Achievements

         Academic potential

         Career goals

         Community and/or campus involvement `

Any other information pertinent to the scholarship(s) for which you are applying.
