Letter of Agreement

The cost of the training program for eligible employees is provided for by the Employment Training Panel (ETP). The following information is to clarify the company’s obligation in obtaining this training for its’ employees.

1. Student and company agrees that students are required to give cancellation notice of 7 days with a binding re-schedule date (2 cancellations per individual program is permitted).

2. Failure to notify CSUDH of a cancellation within the given time will result in a charge to the company for the full cost of the particular training class ($216.00/class day).

3. Failure to complete the "pre-determined" entire course of study by a student will result in the company being liable for the full cost of the student’s entire training program.

4. Prospective ETP funded students must have been employed by the company at least 90 days prior to the start of training. If a student attends training and has not been employed by the company for at least 90 days, the company will be liable for the full cost of the student’s entire training program. (please notify your program administrator - in writing - for an exemption request)

5. Students must be employed by the company for at least 90 days following the last date of training. If a student attends training and is not retained by the company for at least 90 days after the last day of training then the company will be liable for the full cost of the student’s entire training program. (Please notify your program administrator as soon as a separation occurs).

6. Students may not be concurrently enrolled in another ETP funded program; concurrent enrollment in another ETP funded program will result in the company being liable for the full cost of the student’s entire training program. * Note: students may enroll in workplace skills courses without your knowledge; ask your prospective attendees about their current training activities and if questions arise do not fail to apprise us in writing.

7. Company will be liable for cost of training should it leave California within 3 years from the end of training date.
