Maintenance Request
Instructions for making Maintenance Requests
Login using your assigned login name and password, Database name is: LOKER
Once logged in, select 'Maintenance Request Records'
Enter a short description as to the nature of the request
Enter name, contact number, & email address
Enter the 'Type of Work Order', if unsure, leave blank
Enter your location and/or identify the tenant
Select the category of the request
If this is an Emergency, contact the Student Union Admin Office at (310) 243-3854.
In the area provided, give a fully detailed description of the problem. Be as detailed as possible.
When completed, click 'Submit'
You will be provided with a Request Number and the ability to print a copy of the request.
Note: To print a receipt. IE pop-up blocker must be disabled.
