Announcements:

There is now a new procedure for submitting Outreach Trainer Reports. Trainers will pay a $5 fee per card (letter sent to CSUDH trainers informing them of upcoming fee). Pay via Paypal using the link above, and then submit your report using the forms below. Please use the new forms when submitting your reports, or you will be asked to resubmit your reports.

Important: if you submitted a report online between September 27, 2011 and November 7, 2011, please resubmit your report via fax at 424-258-7056, or scan the pdf and e-mail it to osha@csudh.edu.
