2008 - 2009 EOP Forms

To apply for fall 2008 admission through the Educational Opportunity Program, applicants must submit the Applicant Information Form and two Recommendation Forms.  If you want to apply online you must first apply for admission to CSU Dominguez Hills on the CSU Mentor website at www.csumentor.com.  After completing the CSU application, you can then access the online EOP application on the CSU Mentor site. 

If you do not want to apply for EOP online, you can access the printable EOP forms and mail them to the EOP office. 

The printable forms are in PDF format.  Therefore, Adobe Acrobat Reader is needed to view and print the forms.  If you do not have Acrobat Reader, click the the icon below to download the software from the Adobe web site.
