After inserting the Form area, you will see a rectangular area outlined with a dashed line with the Submit and Reset buttons at the bottom of the form area.

You can type your page heading and introduction before or after you insert all the form elements.

One-Line Text Box:
On the Insert menu, highlight Form, and click on One-Line Text Box.
 

To configure the text box properties, right click on the text box and click on Form Field Properties...

Create a field name for the text box.  Set the width of the text box.  If Yes is selected for Password field, every character input by the user will be displayed as * on the browser.

If you click on the Validate... button under the Text Box Properties window, you can check the Required box if the user must fill out this text box on the form.  The Display Name is the field name to be shown on the error message if this box is empty when submit.
