Radio Button:

Under the Insert menu, highlight Form, and click on Radio Button.

To configure the radio button, right click on the radio button and click on Form Field Properties.
You can also double click on the radio button to open the properties window.
In the Group Name box, type a name to identify this group of radio buttons.  All the buttons in the same group will have the same group name but with different values.

Check Box:

Under the Insert menu, highlight Form, and click on Check Box.

Double click on the check box to set up the properties.
