Drop Down Menu:

Site visitors can choose from a list of items from a drop down menu.  

Under the Insert menu, highlight Form and click on Drop Down Menu.
Double click on the Drop Down menu to configure the properties.
Click the Add button to add items on the drop down menu.

Click on the Validate... button.
If the Data required box is checked, the site visitors must select at least one of the listed items before submitting the form.
If the Disallow first choice is checked, the site visitors will not allowed to select the first choice.

If the site visitors are allowed to select more than one item on the list, select Yes on the Allow Multiple selections radio button.
