Veterans' Certification Request Form

This form needs to be submitted EVERY SEMESTER that you wish to use your benefits for tuition supplementation.

Optimal processing time for certification submission is seven (7) business days.

IMPORTANT: Changes in your class schedule may affect your VA benefits. Notification of adds/drops at any time during the term is necessary to maintain accurate certification records for your benefits.

Notify the Certification Office for Veterans Affairs of any changes immediately by submitting the form below. Changes in enrollment after the last day to drop/add courses may result in the recoupment of stipends or allowances as seen fit by the U.S. Department of Veterans Affairs.

*Please note, the VA does not cover late fees. If you receive a charge for a late add/drop or processing fee, it will NOT be included in the charges sent to the VA.*

However, the U.S. Department of Veteran Affairs may approve mitigating circumstances involved in the change, if properly notified and in a timely manner. For this reason, we ask that the Certification Office for Veterans Affairs be notified of any and all changes in enrollment.

* Required field. For the Student ID, enter the number as it appears on your application acknowledgement letter or Student ID card. Current students, note that this form requires your new ID number and not those starting with "98".
