 As my VA region's Home Hospice Care Coordinator for the past three and a half

As my VA region’s Home Hospice Care Coordinator for the past three and a halfyears, I have dedicated my work in support of Veterans facing end of life. Previous to that, I spent seven years as a Home Based Primary Care Case Manager, visiting geriatric Veterans and families in their homes in Oakland, California. Many of these Veterans are palliative care patients with oncology diagnoses and other multiple complex chronic health and behavioral health conditions. Then and now, I work closely with community hospice agencies who serve Veterans’ end of life care needs at home. Through my graduate nursing studies and my experiences as a VA nurse, my leadership skills have continued to grow. In my daily work, I frequently educate other staff about the care of Veterans at end of life, and I have presented about hospice at a regional Nursing Grand Rounds. I am honored to receive this award, and I plan to apply the new knowledge and skills obtained through my graduate degree to improve and expand support of end of life care for our Nation’s Veterans.
