If you are taking online-only classes, you can select the option that states that you will not be accessing campus
ALL CSUDH students, regardless of on-campus or online status, must complete the COVID-19 vaccine self-certification form upon registration in CCPE courses. Please note that this does not necessarily mean that you have to be vaccinated in order to take CCPE courses. Please see details below.
What does completing my vaccine self-certification mean?
Self-certification means a student certifies and submits appropriate documents through MyCSUDH (Campus Portal) to prove that they either:
Once you register for a class, CCPE Registration will provide guidance on how to activate your account. If you have recently enrolled and need instructions on how to activate your account, please call 310-243-3741 or email ceereg@csudh.edu and our registration team will email you step-by-step instructions.
Failure to complete self-certification may result in being prevented from registering in Spring 2022 courses. For further information, visit https://csudh.edu/ceie/covid.
If you don't know or have forgotten your login credentials, including your student ID number, please call 310-243-3741, option 1 during business hours.

To complete your self-certification, log into my.csudh.edu. In the left navigation, select COVID-19 Vaccination. In the drop-down option, select COVID-19 Vaccine Certification.
