COVID Information for Online CCPE Students

COVID-19 Self-Certification Required

If you are taking online-only classes, you can select the option that states that you will not be accessing campus

ALL CSUDH students, regardless of on-campus or online status, must complete the COVID-19 vaccine self-certification form upon registration in CCPE courses. Please note that this does not necessarily mean that you have to be vaccinated in order to take CCPE courses. Please see details below.

What does completing my vaccine self-certification mean?
Self-certification means a student certifies and submits appropriate documents through MyCSUDH (Campus Portal) to prove that they either:

  1. have been fully vaccinated against COVID-19, including booster;
  2. qualify for a medical exemption;
  3. qualify for a religious exemption; or
  4. will NOT access the campus for any reason

Instructions: How to Self-Certify

  1. To complete your self-certification, log into my.csudh.edu.
  2. In the left navigation, select COVID-19 Vaccination.
  3. In the drop-down option, select Verify COVID-19 Vaccination.

Special Notes

  • Most CCPE courses are now online. If you are only taking online courses and do not plan to access the campus, you can check the fourth option.
  • Boosters must be administered by February 28, 2022, or six-months after the final dose of the original vaccination, whichever is later.
  • All students with approved medical or religious exemptions must participate in surveillance testing if they are enrolled in any in-person or hybrid courses. More information is available on the CSUDH Testing Information site.
