State employees (faculty, staff, student assistants, and special consultants) and registered volunteers who drive a university or privately owned vehicle, or powered cart must submit the forms below, complete Defensive Driving training, and must be enrolled into the California Department of Motor Vehicles (DMV) Employer Pull Notice (EPN) program.

Completing Defensive Driver Forms

All drivers must possess a valid California Driver’s License. 

1. STD 261

If driving a personal or rental vehicle, complete form STD 261- Authorization to Use Privately Owned Vehicles on State Business. Fill out all fields. 

  • Initial: Employee completes Section 1 (Certification) and Supervisor completes Section 2 (Approval).
  • Renewal: Employee & Supervisor completes Section 3 (Renewal) every year if authorization remains.
  • Submit form STD 261 to Risk Management via Drop Box

2. INF 1101

Complete INF 1101 – Authorization for Release of Driver Record Information (EPN Form). This form will be provided by Human Resources to eligible employees as part of the onboarding process. For existing employees whose job duties change or who now need to drive for “one-off” reasons, complete a copy of the form and submit to Risk Management via Drop Box

  • Employee completes Section 1 (Driver Information). EPN Administrator completes Section 2 (Authorized Representative Certification).
  • Per CA DMV, the form will take a minimum of (7) working days to process.

3. CSUDH Volunteer Identification Form

  • Volunteers Only. 
  • Complete CSUDH Volunteer Identification Form. Volunteers should register with Human Resources.
