To be considered fully immunized against SARS-CoV-2, you must have received either the approved two dose vaccine or the approved single dose vaccine and the SARS-CoV-2 booster. The deadline to receive the booster is six months after receiving the final dose of the original vaccine. Students who are not fully vaccinated (including completing the self-certification process with vaccination record upload), do not have an approved medical or religious exemption or selected no physical access to campus are not able to access campus facilities.
CSUDH requires students who are accessing university facilities or programs to receive a vaccine booster shot to be fully immunized against SARS-CoV-2, the virus that causes COVID-19, and in compliance with the university's COVID-19 vaccination policy. Boosters must be received by six months after an individual received the final dose of the original vaccination.
You may be excused from the vaccine requirement due to a medical condition for which an Approved Vaccine presents a significant risk of a serious adverse reaction. Any medical exemption must be verified by a certified or licensed healthcare professional. You will need to complete the self-certification process, indicate medical exemption, and provide the appropriate documentation.

If you are not yet eligible for the booster, but you have already completed the original self-certification process, you should wait until you are eligible to receive the booster. Once you are eligible, you should then get the booster and complete the self-certification process to include the booster information.
If you are not eligible for the booster now, you should wait to update your self-certification record until you are eligible to receive the booster. Once you are eligible and receive the booster, you will need to update your record within your myCSUDH portal.
The CSU's vaccine requirement includes the SARS-CoV-2 booster. If you received the original approved vaccination and are eligible to receive the booster based on the time of your last vaccination, but do not plan to get the booster, you must resubmit the self-certification and qualify for a medical or religious exemption.

Once you submit the self-certification, you will receive a final message asking you to click "Finish" to return to the homepage. You will be redirected to the self-certification page once you click "Finish." Do not resubmit the documentation unless you are instructed to by a member of the CSUDH COVID-19 Response team. Resubmitting the certification multiple times may cause a delay in your record being processed.
All certifications and exemptions are reviewed and approved by designated officials. All vaccination documents are verified for authenticity and exemption requests are approved on a case-by-case basis, which requires 3-5 business days. You will receive a notification on your ToroMail regarding the approval status of your vaccine certification or exemption request. Vaccine Self-Certification submission on myCSUDH is not an automatic approval.
To check on the status of your self-certification, or to inquire about the self-certification hold, you should call the COVID Safety line at (310) 243-2809 or email covid_safety@csudh.edu. Always include your first name, last name and CSUDH Student ID number when emailing the university.

You may be able to find virtual classes to replace those meeting face-to-face, either because it's an online section of the same course, or because it’s a different course, offered online, that meets the same degree requirement. However, it’s in your interest to come to campus if that’s how your classes are offered. Most of our courses and instruction are optimized for face-to-face instruction. For these courses you’ll find the subjects make more sense, and that it’s easier for you to remember what you've learned and apply it in other settings, when you take your classes the way they were designed.
In the case of COVID-19 or any other medical emergency that keeps you away for some of the semester, we will work with you to minimize the impacts to your learning, your grades, and your progress to graduation. If you have to quarantine, then make sure you immediately notify your instructors and the Office of the Registrar.
CSUDH is taking all necessary measures to ensure safety requirements will be met for in-person classes. This includes enhanced cleaning and mandatory mask-wearing in classrooms and laboratories.
Students who don't hear back from their instructors should next contact the chair of the relevant department. If there is still no response, contact the associate dean of the college.
