To be considered fully immunized against SARS-CoV-2, you must have received either the approved two dose vaccine or the approved single dose vaccine and the SARS-CoV-2 booster. The deadline to receive the booster is Feb. 28, 2022 or six months after receiving the final dose of the original vaccine, whichever is later. Students who are not fully vaccinated (including completing the self-certification process with vaccination record upload), do not have an approved medical or religious exemption or selected no physical access to campus are not able to access campus facilities.

The CSU's vaccine requirement now includes the SARS-CoV-2 booster. If you received the original approved vaccination and are eligible to receive the booster based on the time of your last vaccination, but do not plan to get the booster, you must resubmit the self-certification and qualify for a medical or religious exemption.

If you received a medical or religious exemption, you are required to test weekly if you are accessing campus facilities. If you received the exemption, are enrolled in a fully online course load, and do not access campus facilities for other purposes, you are not required to complete the weekly testing.
Certain student populations are required to test weekly, including those who have received a medical or religious exemption, student athletes and students who are living in University Housing. If you fail to complete the mandatory weekly testing, you may be subject to the Student Conduct process with disciplinary action. We encourage all students who are required to test weekly to complete the requirement every week.
If your schedule makes it difficult to get tested on campus testing days, you may get tested elsewhere and submit your weekly result via Dropbox. When submitting the file, save the image/document with your first name, last name and CSUDH Student ID number to ensure the record is processed correctly.
Once you submit the self-certification, you will receive a final message asking you to click "Finish" to return to the homepage. You will be redirected to the self-certification page once you click "Finish." Do not resubmit the documentation unless you are instructed to by a member of the CSUDH COVID-19 Response team. Resubmitting the certification multiple times may cause a delay in your record being processed.
All certifications and exemptions are reviewed and approved by designated officials. All vaccination documents are verified for authenticity and exemption requests are approved on a case-by-case basis, which requires 3-5 business days. You will receive a notification on your ToroMail regarding the approval status of your vaccine certification or exemption request. Vaccine Self-Certification submission on myCSUDH is not an automatic approval.
To check on the status of your self-certification, or to inquire about the self-certification hold, you should call the COVID Safety line at (310) 243-2809 or email covid_safety@csudh.edu. Always include your first name, last name and CSUDH Student ID number when emailing the university.
Yes, if you originally self-certified as not accessing campus, but you are now enrolled in face-to-face courses, you must update your self-certification record to be in compliance with the vaccination requirement.
If you are receiving an error message, or if you have been told by someone on the COVID-19 Response team that your document was not accepted, you must resubmit the document within the proper guidelines. Save and upload any supporting documents on myCSUDH as .bmp, .gif, .jpg, .jpeg, or .png. PDFs are not accepted. If the image is too large, you will need to resize the image to ensure the system is able to register the file.

If your vaccination records are on two different files, take an image that includes both records within one photo. You can do this by placing the two images side-by-side and taking a single photo of both records together.
