A licensed healthcare professional must complete the form that you have an established treatment relationship lasting at least one month.

The Office for Students with Disabilities will review your application and documentation to determine if it is appropriate and complete.
Schedule an Intake Appointment
A Disability Management will reach out to request an appointment to discuss your housing accommodation request. Accommodation requests are processed in the order they are received and are evaluated on a case-by-case basis.
The housing committee will assess the request after the necessary documentation and intake appointment have been finalized. The committee will convene monthly and address emergencies on a case-by-case basis.
Requests will have to be renewed every academic year.
Please Note:
Emotional Support Animal (ESA) are not allowed in DH Housing until the request is approved by the SdRC Housing Committee.
Housing accommodations are provided based on the availability of rooms/space and the requested housing accommodation(s) are not guaranteed
The SdRC is not responsible for any housing fees as a result of accommodation approval. Fees are assessed and determined by University Housing
